Client Onboarding Portal
The clinic needed a structured way to collect patient health histories, consent forms, and lifestyle questionnaires before the first appointment. Paper forms were often incomplete, and staff spent too much time chasing missing information. The goal was to build a simple digital portal that patients could complete at home, on any device, before arriving at the clinic.
The portal had to handle sensitive health data securely, integrate with the existing practice management system, and present a clear, calm interface that matched the clinic’s natural aesthetic. No complex dashboards or analytics — just a straightforward intake flow.
Approach
We started by mapping the existing paper forms into logical sections: personal details, medical history, current medications, lifestyle habits, and specific concerns. Each section was broken into short, single-topic steps to avoid overwhelming the patient. Progress indicators and a save-and-resume feature were added for longer forms.
The design used the clinic’s sage green and warm beige palette, with soft rounded buttons and generous spacing. All text was set in a clean sans-serif typeface at a comfortable reading size. No medical jargon — every label and instruction was reviewed by the clinic staff for clarity.
On the technical side, we built a lightweight backend that encrypted data in transit and at rest. The portal connected to the clinic’s scheduling system via a simple API, so completed forms automatically triggered a confirmation email and a notification to the assigned practitioner.
Implementation Details
- Six-step intake form with conditional logic (e.g., if a patient reports a chronic condition, additional fields appear)
- Secure file upload for previous lab reports and medication lists
- Digital signature field for consent forms, compliant with local health data regulations
- Automated reminders via email and SMS for incomplete forms, sent 48 hours and 24 hours before the appointment
- Admin dashboard for staff to review submissions, flag incomplete entries, and export data to the clinic’s EHR
Outcome
Within the first three months, the portal processed over 200 patient intakes. The rate of incomplete forms dropped from 35% to under 8%. Staff reported saving roughly 4 hours per week on follow-up calls and data entry. Patients appreciated being able to complete the forms at their own pace, and several commented that the process felt “less clinical” than paper forms.
The clinic now plans to extend the portal to include follow-up symptom tracking and post-consultation care plans, building on the same simple, patient-first approach.
